NAVI MUMBAI: A 34-year-old woman with a high-risk pregnancy gave birth to triplets at Medicover Hospitals, Navi Mumbai.
The medical team consisting of consultant gynaecologist Dr Rohini Khera Bhatt, consultant paediatrician and neonatologist Dr Swapnil Patil, paediatrician Dr Vrukshal Shamkuwar, and anaesthetist Dr Jayashree, successfully managed the high-risk triplet pregnancy.
Mrs Rita Kumar (name changed), a resident of Vashi and a homemaker, was able to achieve her first pregnancy through IVF and was carrying triplets. The couple had been trying natural pregnancy for a long time but with no results. Such triplet pregnancy is rare and high-risk because the body changes that are expected in the mother carrying a single baby are multiplied in this case. Also in this pregnancy, two out of three babies were sharing one placenta.
At 28 weeks of her pregnancy, she informed Dr Rohini Khera Bhatt of suffering from abdominal pain which she had been experiencing for the last 2 weeks and was not getting relieved. An ultrasound was done which revealed that her cervix was too short and beginning to open despite the cervical stitch which was done as a preventive procedure at her IVF Centre in Navi Mumbai.
It was decided to prolong the pregnancy as much as possible by bed rest, medications to prevent contractions, corticosteroids to help the babies’ lungs mature, antibiotics to prevent any infection because of decreasing cervical length, and regular monitoring of both the mother and the babies. The pregnancy was extended to 34 weeks.
The best outcome for such triplet pregnancies is expected to be at around 34 weeks because the risks of prematurity complications in babies are significantly decreased and the risks of prolonging pregnancies in babies sharing a common placenta are also taken care of. A Caesarean section was performed because of the complexity of the case. The woman delivered healthy babies, that is a baby girl weighing 1.59 kg and Identical Twin Boys weighing 1.75 kg and 1.43 kg.
All three babies were admitted to the NICU under the care of Dr Swapnil Patil for close monitoring. The second baby had breathing difficulty for which he was kept on respiratory support for 4 days. Kangaroo mother Care was started for the babies by both parents from day three onwards. The babies were discharged after 2 weeks.
All three infants were active, had stable vital signs, and were discharged on exclusive breastfeeding. Regular pediatric check-ups were scheduled to monitor the growth and development of the infants. The first follow-up visit has been done and the babies are doing well.
With close monitoring, timely medical intervention, and supportive care, the mother and her triplets had a positive outcome. “The handling of complex triplet deliveries sets a new standard in maternal care.
