Inquiry Report Pins Blame on Patna Metro Agency for Fatal Tunnel Accident | Patna News – Times of India



Patna: The inquiry report submitted by a three-member committee formed by Patna district magistrate Chandrashekhar Singh into the metro tunnel accident has held the agency responsible for negligence, leading to two spot deaths and another worker succumbing to his injuries during treatment.
The committee found the absence of proper maintenance and upkeep of equipment and disregard for safety measures and monitoring.
The DM, in his letter to the secretary of urban development and housing department on Wednesday, called for strict action against those responsible for the tragic accident.
The committee, in its report, said that the agency responsible for the Patna metro project did not follow the safety norms during tunnel work as per standard operating procedures (SOP) and lacked proper monitoring of the same. “The agency did not follow the norms of tunnel safety and health plan, which required daily brake checking by loco pilot and weekly loco engine servicing and record-keeping of the same,” said the report.
It added that the agency also failed to provide the third-party inquiry (TPI) report following checking of normal and emergency brakes of the loco machine.
During investigation, the committee found that it has not got itself registered under Article 4 of the Inter-State Migrant Workmen (Regulation of Employment and Conditions of Service) Act, 1979, which underlines the safety and welfare of migrant workers, despite employing many such workers.
The accident took place on the night of October 28.
